I’m using something called Did You Pass Math. You pick it up here: http://www.herod.net/dypm/
I’d recommend turning off Akismet when you activate it, though. Otherwise users will have trouble commenting on return visits.
Also, there’s at least one bot that can get by it. I’ve had it happen on two blogs. I’m thinking of asking to get let in on the private Beta of Defensio (http://defensio.com/), but spam isn’t really a problem for me here yet.
